If you go to a hospital emergency department instead, you will spend hours behind people who are in a worse state than you are, and that is exactly as it should be.

Distance here means distance across a wide catchment

The city itself is not large, but the area attached to it is wide and the surrounding towns connect by regional train. The group registered here is small, so restricting the search to the city boundary narrows your options for no good reason.

For a specialty you see once or twice a year, half an hour on a train is tolerable. For weekly treatment it is not. Apply that test and decide from it.

Hannover is close by train and has a longer list, so it is the natural next step. Phone before travelling to ask whether they take new patients and whether a referral is needed. If you have a GP, ask them too, because they usually know which colleague has worked with Persian-speaking patients.
