About Persian-speaking Urologists
Urology is the specialty people put off longest, and rarely because of the waiting list. It is that the appointment requires saying something out loud: about urine, about the genitals, about sex, about a body that no longer does what it did. In a second language that sentence has two versions, a clinical word nobody taught you and a street word you will not use in front of a stranger, so many people say neither and wait another year. Two things change that, and neither is medical: a doctor you can say it to in Persian, and, if it matters to you, a doctor of the same gender as you.

Not only a men's doctor
Urology covers the whole urinary tract, kidneys, bladder and urethra, and alongside that the male reproductive organs. The first part belongs to everybody: stones, blood in the urine, infections that keep coming back and a bladder that will not hold are urological questions for women and men alike, and children have their own version of the field. A woman's bladder problem can sit next to a gynaecological one, and practices differ on which handles what; Gynecologists have their own list here.

You can book without going through anyone first
With statutory insurance a urologist is normally a direct booking, and no referral is required first. For many readers that is the most useful line here, because the conversation they dreaded was the one at the family practice. Take a referral if you have one; not having one is no reason to postpone, and you do not owe the receptionist your symptoms.
The check-up that no letter arrives for
Men on statutory insurance have an annual cancer screening entitlement at the urologist from a set age, usually given as 45. What is worth knowing is how it differs from the other programmes: for bowel and breast cancer your insurer writes and invites you, and for the prostate it does not. No letter, no reminder, no appointment waiting. It exists only if you claim it. Your insurer publishes what you are entitled to and at what age, and that list is the authority.
Alongside it you will often be offered blood-marker tests as a self-pay service. That is lawful and normal, but it is a different pocket: where there is a documented reason to suspect something, an insurer may cover the very test you would otherwise pay for. So before anything is taken, ask which part of today goes to your insurance and which part you are paying for, and why this test, for you. A self-pay service should be agreed in writing beforehand.
If this is about having children
The man's half of a fertility investigation, a semen analysis and some blood work, is done here, and is usually quicker and less invasive than the woman's. Finding out why a pregnancy is not happening is generally covered by your own insurer. The treatment that may follow is a separate question with strict conditions: the statutory rules take account of marital status, the age of both partners and a limited number of attempts, and require a treatment plan the insurer approved before anything began. What the insurer contributes is a share, not the bill, so get your insurer's answer in writing before the first invoice exists.
Vasectomy, and changing your mind later
Sterilisation for contraception is generally not a statutory benefit in Germany, and private insurers usually will not cover it either, so treat it as your own money. Reversing it later is the same, and costs considerably more. Because you are paying, ask in writing for the total, for what the consultation and anaesthetic add, and for which follow-up checks are included. A practice that will not put a total on paper has told you something.
Stones, infections, and the one nobody brings up
Kidney stones and repeat infections get seen, because they hurt. Continence is the opposite. Leaking urine is far more common than the silence around it suggests, it happens to women and men, it is not simply part of ageing, and a great deal can be done about it. It is also the complaint people are least likely to raise: it comes out when a doctor asks directly, and a doctor who cannot ask in a language you answer honestly in does not get the answer.
Getting ready, and asking for a woman or a man
Most first appointments involve a urine sample, and the instructions differ: some practices want you to arrive with a full bladder, some want you not to have passed urine for a couple of hours. Ask when you book, because arriving having just emptied your bladder can turn one visit into two.
Ask in the same call whether a female or a male doctor is available and on which days, because a practice with several doctors may have only one. This is an ordinary request and practices hear it constantly; there is no filter for it here, so it must be spoken.
